Good morning, Baltic Bulldog students and parents,
I wanted to remind you that summer activities have started and remind you that health history and physical forms are required for Baltic activities for the Fall, Winter, and Spring sports seasons and activities. We recommend that you schedule a physical with your primary care physician for your student-athlete once a year and turn in the proper documentation to the Baltic School District per our athletic policy. This is for grades 7 through 12 in the 24-24 school year. For example, if you turned in a completed physical on 03-09-24 your student-athlete would be cleared to play Baltic activities until 03-09-25. Then a new physical and health history form would be required before the student-athlete could continue activities. Please reach out with any questions you might have.
Attached is a copy for you to print and complete with your primary care physician. Please note that there are two pages and a couple of signatures required from both the parent/guardian and the student, so please don't forget those signatures before turning the forms into the school office or activities director.
